Staffordshire EWB Service Consent and Privacy
CONSENT OF CLIENT/YOUNG PERSON (to be completed if young person and/or parent legal guardian is completing)
I/We have had the Action for Children Service explained to us. I/We confirm consent to the above referral and any assessment that may be required. By consenting to this referral, the Young person / Parent / Carer agrees to information being collated about the person(s) named on this form and the information on this form. I/We give permission to the Support Worker’s/clinicians from Action for Children to contact other agencies for further information in relation to this referral. I/We also agree that information held by Action for Children and obtained through the referral and assessment process may be shared with relevant agencies or organisations for the purpose of developing and implementing a support plan. Information may also be shared with outside agencies for the purpose of evaluating the effectiveness of Action for Children’s involvement both locally and nationally. The information provided on this form will be processed by Action for Children in accordance with the General Data Protection Regulation and the Data Protection Act 2018.
Sharing your personal information with Staffordshire County Council
Personal information which you supply to Action for Children may be shared with Staffordshire County Council departments and their partners if they supply a service directly to you. This is to ensure that you receive the best service available as it will improve communication between organisations. These partners can include Health, Schools, Voluntary sector, department for work and pensions and the Police. Staffordshire County Council will not disclose your information to any partner who is not supplying a service to you or any other organisation unless required to do so by law. If you have any concerns about the planned use of your information, please speak to your Support Worker. For further information on how your information is used, how we maintain the security of your information and your rights to access information, please refer to the Privacy Notice provided to you by your Support Worker/clinician.

I have discussed the Action for Children Services with the young person/family and they agree to this referral and understand family information is needed to process the referral. Yes No The child or young person whose data is going to be used (processed) should consent to this wherever possible. A parent or guardian should usually only give consent on behalf of the child or young person where the child or young person does not have an understanding of what it means to exercise their rights under the General Data Protection Regulation. Generally, children aged 12 or under will often not have sufficient capacity to give consent, therefore, a parent or guardian should consent on their behalf, but each individual, their abilities and the type of personal information that is going to be processed should be considered independently.
